NYSPHSAA Communicable Skin Disease Form Must be filled out by a physician if a wrestler has a communicable skin disease i.e Ringworm. Must be fully completed and on file with the coaches for wrestler to participate.
WCSD Parental Transportation Release Must be completed and on file with the coaches in order for wrestlers to not take WCSD transportation to and/or from events.
